Beginning in 1780, the list, originally published in paragraph form, gives the original name, the name change, the county or city of residence, and in some cases the reason for the name change. Beginning in 1851 the list was separated by county. Each listing includes the original name, the name change and the city of residence. Counties include Suffolk, Essex, Middlesex, Worcester, Hampshire, Hampden, Franklin, Berkshire, Norfolk, Bristol, Plymouth, Dukes, Barnstable and Nantucket. Name changes as a result of adoption, the reason for the majority of changes, are indicated by an asterisk. Both an original name and an adopted name index are included.
